Some of NextLevel Health’s most important benefits include:
- A Care Management Team that can come to you
- Help setting up a doctor’s visit or mental health appointment
- Providing a ride to and from your doctor’s appointment. Call from 8 a.m. to 5 p.m. at least two days in advance of your appointment
- After hours medical assistance. Call to talk with a nurse after 5 p.m. and up until 8:30 a.m. if you cannot reach your doctor
- Health tools, like weight scales and blood pressure and diabetes monitors
- There are no copays for our Members
- Help to understand Medicaid rules for prescriptions
- Support finding housing, diet programs, and financial assistance
- Assistance with Medicaid redetermination to make sure you do not lose your benefits
