NextLevel Health is a Medicaid managed care health plan for people with Medicaid, but not Medicare, who live in Cook County. We assist you in managing and coordinating your care with a deep understanding of you, your community, and your health—one on one, right in your own community.
We are attentive, responsive, respectful, compassionate, and mindful of the diverse needs of our members and providers. We focus on diverse populations that make up the larger part of our local community. We help establish, maintain, and increase access to personalized health care services in the areas we serve through our ongoing, active, and compassionate presence. We are a consistent presence in the community, always available when our neighbors need us most.
Our vision is to help Cook County’s Medicaid-reliant residents achieve a healthy lifestyle and enable them to receive the highest standard of care, with continuous access to a quality Medicaid managed care health plan.
Our role as a Medicaid managed care health plan is enhanced by being active members of the communities we serve. Our leadership is community-centered. We play an active role in our community by improving health care access and increasing public awareness. Because when our members are healthy, their families grow stronger and our entire community thrives.
A Healthier You. A Healthier Community. NextLevel Health serves our community by making it a healthier place. We speak honestly about our community because we have deep roots here. We were founded in Cook County, Illinois, so we are truly “home grown.”
NextLevel Health employees live and work in the community too. We develop lasting relationships with local businesses. We are as deeply embedded in community culture as a barbershop or corner store. Because we are an integral part of the local community, we have a special understanding of the needs of our members and providers. Our visibility keeps us connected to community health issues and validates the positions we take.
