- Look for a Member Welcome Packet in the mail. It includes your Member ID card and Member Handbook. If you have not received it within 10 days, call between 8 a.m. and 5 p.m. to request another packet.
- Make sure the doctor’s name on your card is right. If it is wrong, call between 8 a.m. and 5 p.m. to receive a corrected card.
- Complete the Health Risk Screening. Return it to us by mail, fax, or email. You can also complete the screening by calling between 8 a.m. and 5 p.m. We can help you from there.
- Make a doctor’s appointment to review your health goals. Your Care Management Team can help you make that appointment and answer any questions.
