My First Steps

To enjoy everything NextLevel Health has to offer, new Members should:

  • Look for your Member Welcome Packet in the mail. It includes your Member ID card and Member Handbook. If you have not received it within 10 days, call 1-844-807-9734 between 8 a.m. and 5 p.m. to request another packet.
  • Make sure the doctor’s name on your card is right. If it is wrong, call 1-844-807-9734 between 8 a.m. and 5 p.m.
  • Complete the Health Risk Screening. Return it to us by mail, fax, or email. You can also complete the screening by calling 1-844-807-9734 between 8 a.m. and 5 p.m. We can help you from there.
  • Make a doctor’s appointment to review your health goals. Your Care Management Team can help you make that appointment and answer any questions.

Meet Your Care Coordinator

This person will help you with your medical needs. He or she can help you make a doctor’s appointment and get you to and from appointments.
