I am qualified for Medicaid! How do I become a member?
We are a Medicaid managed care health plan for people on Medicaid, but not Medicare, who live in Cook County. We are hired by the Illinois Department of Healthcare and Family Services. Visit About Us.
All of covered services are FREE. Our Members pay nothing because Medicaid covers the costs. NextLevel Health has no copays. Click here for a complete list of benefits.
You must live in Cook County and be on Medicaid, but not Medicare. During your open enrollment period, you can choose NextLevel Health. If you do not know when your open enrollment period is, or if you have questions about enrolling, contact Client Enrollment Services.
Contact Client Enrollment Services:
Go to enrollhfs.illinois.gov to sign up. Choose a health plan, a PCP and review your choices.
You’ll also need:
Fill out the enrollment form you received from the Department of Healthcare and Family Services and send it back to them in the envelope they provided.
